Information we collect
When you submit a form, we may collect your name, email address, optional phone number, state, ZIP code, selected panel, requested quantity, organization details for provider inquiries, and the message you choose to send. Our website and hosting providers may also process routine technical information, such as IP address, browser or device type, timestamps, and security logs.
Seven-question test panel guide
The test panel guide processes your selections temporarily in browser memory. It does not require an account or contact information, does not store the answers in local or session storage, and does not place answer text in the page URL. Answers clear when you restart, leave, or reload the page. Only a panel you separately add to the request cart may persist for the current browser session; the health-related answers that led to it do not.
Do not submit health information
Do not include symptoms, diagnoses, medications, medical history, laboratory results, or other health information in a website form or email. These public channels are not intended for medical records or clinical communications.
How we use information
We use submitted information to respond to requests, prepare nationwide shipping, order, or program details, provide support, protect the website, and meet legal obligations.
